Course Objectives
In the rapidly changing landscape of ageing, health and social care are often intertwined and plays a pivotal role in enabling citizens to age in place. The Specialist Diploma in Integrating Health & Social Care goes beyond general knowledge by equipping participants with the skills and strategies needed to address the complex challenges of managing health and social issues. Participants will have confidence to design and implement innovative programs, services, and interventions tailored to the unique needs of individuals and communities.The proposed course is designed for professionals and mid-careerists seeking to advance their careers and contribute to the holistic well-being of individuals and communities.
Course Description
The Specialist Diploma in Integrating Health & Social Care (SDIS) is a part-time CET course, consisting of the following modules:1) Social Prescribing & Asset-Based Community Development (45hrs)
2) Promoting Positive Health Behaviours to Support Patients and Caregivers (45hrs)
3) Case Management in Health & Social Care (45hrs)
4) Programme Management in the Community (45hrs)
5) Empowering Community through Volunteer Management (45hrs)
6) Group Dynamics and Facilitation (45hrs)

Refund and withdrawal policy
- Please note that a 100% refund will be available if the withdrawal request is submitted more than or equal to 14 days before the course start date.
- Please note that a 50% refund will be available if the withdrawal request is submitted less than 14 days before the course start date.
- Please note that no refund will be available if the withdrawal request is submitted on or after the course start date.
